Households with registered Fitchburg voters and/or persons already listed as residents in Fitchburg will automatically receive a census form each year in January or February.  If you have recently moved to the City and/or did not receive a census form please contact this office at 978 829-1820 to request that one be mailed to you.  Responding to the annual city census maintains your active voter registration and/or residency status.

Birth, Marriage and Death Certificates

Long form (includes all information required for passport or legal purposes) $10.00

To order by mail: Send a short letter* to the City Clerk, 166 Boulder Drive, Fitchburg MA 01420 stating name and date of birth, marriage or death of the person(s) for whom you are requesting the certificate, a check or money order for $10 payable to the City of Fitchburg for each certificate that you are requesting and a self-addressed, stamped envelope so that we can mail it back to you.

*NOTE:  If the parents were not married at the time of the child's birth, the birth and/or marriage certificate for that child is not a public record.  Therefore, only those persons whose names appear on the certificate may obtain a copy and that person's signature on the letter must be notarized.

Marriage Intentions

Filing fee $25.00

Both parties must appear in person together before 4:00PM to file intentions. Please bring with you a photo identification and, if applicable, proof that your divorce is absolute if your divorce occurred within the last year.  You must be married in Massachusetts within sixty days of filing.  You must pick up the license before the wedding ceremony and present it to the person who is officiating. There is a three day waiting period between the time you fill out the paperwork and when you can pick it up.  A district or probate court may issue a waiver of the three day waiting period.  We suggest filing your intentions about two weeks prior to the wedding.

The City Clerk is a Justice of the Peace and is available to perform marriage ceremonies by appointment.

Business Certificates (DBA)

Filing fee $35 for a four year certificate

All businesses other than corporations or individuals doing business in their own name or corporate name must file a business certificate with the City Clerk.  Business owners must first visit the Building Department to obtain permission to operate their business at a specific location. 

Dog licenses

Annual Fees:   Male and female dogs $15.00

                        Spayed female or neutered male dogs $9

All dogs must be licensed and should wear the tag.  Current rabies certificate is required for licensing.  A late fine of $15 will be applied for dogs not re-licensed by May 1st in addition to the prior annual licensing fees not paid.
